A Preservation Rhinoplasty Concept for Achieving a Natural Nasal Bridge
When people think of rhinoplasty, they may imagine procedures such as making the nose higher, refining the nasal tip, or shaving down a dorsal hump. However, the beauty of the nose is not determined simply by height or narrowness. What matters is how naturally the nose can be refined while carefully assessing the thickness of the skin, the strength of the cartilage, the shape of the nasal bones, and the overall balance of the face.
The Let Down technique is one form of Preservation Rhinoplasty. Rather than simply shaving down the protrusion of the nasal dorsum, this approach aims to reposition the nasal bridge into a more natural contour while preserving the structure of the nasal bones and cartilage as much as possible.
In conventional hump reduction, the protruding part of a hump nose is commonly shaved down to reshape the nasal bridge. Depending on the amount of reduction and the patient’s skeletal structure, however, this can sometimes create irregularities, a step-off, or an overly flat appearance along the nasal dorsum.
By contrast, the Let Down / Push Down concept is characterized by its ability to adjust the overall nasal line while maintaining the natural continuity of the nasal dorsum.



A Technique Performed by a Limited Number of Clinics in Japan
The Let Down technique is still performed by relatively few clinics in Japan.
Many clinics offer common rhinoplasty procedures such as hump nose correction, hump reduction, and nasal bone osteotomy. However, clinics that clearly incorporate techniques such as the Let Down technique, Push Down technique, or Preservation Rhinoplasty to refine the nasal bridge while preserving the nasal structure remain limited in Japan.
This is because the Let Down technique is not simply a procedure that shaves bone. It is an advanced surgical technique that requires a three-dimensional understanding of the balance between the nasal bones, septum, and cartilage, as well as precise millimeter-level adjustments.
To refine the shape of the nose while preserving its internal structure, deep knowledge and delicate surgical skill are required in osteotomy, cartilage manipulation, and septal management.
Applying the Concept of TEO Rhinoplasty
At MSA, we apply the concept of TEO rhinoplasty when appropriate, depending on each individual case.
When refining the nasal tip and nasal dorsum, it is important not only to create height or narrowness, but also to consider the support of the entire nose, the thickness of the skin, and the volume of the soft tissue.
This is especially important in patients who naturally have a well-developed, fleshy, and voluminous nasal tip. If the nose is narrowed excessively, the result can appear unnatural. For this reason, we carefully refine the direction of the nasal tip, the flow from the nasal dorsum to the nasal tip, and the balance of the profile while preserving the necessary thickness.
In some cases, the high strip of the nasal septum may be adjusted by approximately 1–2 mm.
Although this is a very subtle maneuver, it can affect the drooping tendency of the nasal tip, nasal support, and the connection between the nasal dorsum and the nasal tip. Therefore, it is an important point in achieving a natural-looking result.
Toward a Natural Result That Does Not Look Overdone
